Privacy Statement

This online privacy statement is intended to inform you of the ways in which this website collects information, the uses to which that information will be put, and the ways in which we protect information you choose to provide us. This site also acts as the login to Qualtrics Survey Tool. Please note that other Michigan State University websites may adopt different privacy practices as their specific needs require. Use of this site is subject to all applicable state and federal laws, as well as general university policies.

The Qualtrics online privacy statement covers the use and disclosure of personal information that may be collected anytime a user interacts with Qualtrics. Such interactions include visiting any of Qualtrics websites, using the service, or when calling the Qualtrics sales and support departments. Information Collected

This MSU Qualtrics site collects two general types of information: (a) information you voluntarily provide through the site, which may include personal information (such as your name, address, email address, etc.), and (b) information MSU and Qualtrics collects automatically when you log into the site and through cookies, third party tracking technologies, and server logs.

Qualtrics collects information for the purpose of software licensing, billing, and practices related with the MSU license. Some information may be used to notify registered service users about new features, special announcements, and events. Users may opt-out of these communications.

Qualtrics does not sell or make available any information about customers or their data without their expressed permission per contractual agreement except as might be required by a law enforcement agency in regards to copyright/patent violations or unlawful acts. Qualtrics maintains a database of user information, which is used only for internal purposes such as technical support and notification of changes to the service or license.

Information Voluntarily Provided by You

This site will only collect personal information that you knowingly and voluntarily provide by, for example, logins, responding to surveys, completing membership forms, sending emails, etc. It is this site's general practice to use personal information only for the purposes for which it was requested and any additional uses specifically stated on the site.

However, information collected through this site may be subject to the Michigan State University's obligation to respond to subpoenas, court orders, discovery requests, and requests for public records under the Michigan Freedom of Information Act. Further, the disclosure of information collected through this site is subject to the provisions of the Family Educational Rights & Privacy Act. MSU may also access or permit access to information collected through this site in accordance with and subject to the limitations of the university's Acceptable Use Policy for Information Technology Resources.

Qualtrics does not sell or rent contact information to other marketers or vendors. Any disclosure of information within Qualtrics (as a company) is strictly to assist with technical or customer service issues. Qualtrics will only disclose customer information as legally required—upon a valid court order under United States or European laws—by law enforcement or governmental agencies for national security or other issues of public importance (for example, acts of war). In any case, customers will be notified as soon as possible if any request is made to disclose their name or any detail of their association with Qualtrics.

Qualtrics University (QUni or technical support) staff may ask for personal information before accessing a user’s account, if the request is by phone or email. However, Qualtrics support staff will never ask for a user’s password. Passwords are salted-hashed values and not viewable by any Qualtrics employee. Further, with the customer’s permission, Qualtrics may access a user’s account to resolve or investigate a software issue within the system or account. If the customer does not wish a Qualtrics support person to access their account, that permission may be disabled by the Brand Administrator.

Information We Collect Automatically

This site and the Qualtrics system collect information automatically through the use of cookies, third party tracking technologies, and server logs. This information does not include any personal information. MSU and the Qualtrics vendor use this information primarily to monitor the use of this site and to make improvements. We may share this data with partners in order to enhance the functionality of the website and system.

Qualtrics collects and analyzes aggregate information of visitors, including the domain name, visited surveys, referring URLs, and other publicly available information. The vendor uses this information to help improve its website and services, and to customize the content on pages for visitors. In addition, Qualtrics reads browser languages and settings in order to customize surveys for respondents.

Cookies

Cookies are small text files used to collect information about website activity. No personal information is collected through our use of cookies. This site uses cookies for two main purposes: (a) to carry information about your current session at this site from one web page to the next, which also allows you to automatically login to other Michigan State University websites using the single sign-on; and (b) to identify you on this site on return visits.

You have the option of disabling or not accepting cookies by changing the preferences on your web browser. However, you will not be able to use any parts of this site or system that require a login.

Qualtrics only uses cookies in the system to maintain the session state, and does not include any response or other personal data.

Third Party Tracking Technology

This site (qualtrics.msu.edu) uses a tracking technology called Google Analytics to obtain site usage information such as users' Internet Protocol (IP) addresses, Internet domain and host names, browser software, and the date and time the site is visited. Data obtained through Google Analytics does not include and is not linked to any personal information. Data from Google Analytics allows us to monitor the use of this site, understand how users find the site, and improve the site's function and content.

Site usage data collected through Google Analytics is not shared with other Google products and tools. Our use of Google Analytics is in compliance with Google's Terms of Service and the MSU Access to Student Information Guidelines. Server Logs

MSU web servers routinely generate logs that contain the following types of information each time the site/system is used:

  • The date, time, and length of your visit.
  • The path taken through our Site and the browser used.
  • The list of files downloaded and the amount of time spent viewing video or audio files.
  • The IP address of the computer accessing our Site.
  • Any errors encountered.

This information is used primarily to monitor the functioning and integrity of the site and system.

Qualtrics Service Users

Qualtrics Service users (those with a unique user account who create surveys) transmit all data to servers via secure Internet connections. Surveys may be designed to collect data anonymously or by specific respondents. In any case, users are responsible for all data they collect. We advise users to be sensitive to privacy concerns, and use survey opt-out options as appropriate.

Brand Administrators control the brand, including authenticated users, survey design, distribution, and collected data. There is an option to require approval before a survey is distributed, thereby enabling a manager (or other designated user) to review before the survey is sent. Qualtrics is not responsible for any data lost or stolen through hacking or negligence by MSU users.

Personal Information Protections

Qualtrics takes preventative measures to protect all customer information, both programmatically and through employee training. All employees must attend yearly security awareness programs (covering privacy, security, and other policies) and sign confidentiality agreements. Security updates and reminders are sent to all employees Quarterly. More detailed information is available in the Qualtrics Security White Paper.

All policy verification is handled through the security and compliance departments. Qualtrics has established internal procedures to review, identify, and track compliance of policies, risk management objectives and regulatory issues. Our Security Officer is a certified member of the International Association Privacy Professionals, and disseminates privacy and regulatory concerns to senior management and the company as a whole.
